## What this record is for

Before a workflow change is relied on, reviewers look at what was checked,
what was found, and what is still open. This sample shows the sections of
that record with example entries, so the structure is inspectable without
pretending any evaluation was actually run.

- Who signs the record, and where the signed copy lives.

A review record earns trust by showing its open questions. A record that
reads as a wall of green checkmarks is telling you less than this page does.
